Nice idea. Maybe it is also better to cover or blur out texts that identifies a company or some confidential identity. and also it would be helpful if we could indicate the root cause if investigation was made and what should have been done or what are the action plans to prevent same incident. a simple format such as stated below may be useful:

Causes:

Preventive Action

:ugeek: I received an e-mail from a friend of mine with this attachment.
:arrow: I wanna share it to all of you.
:idea: As we all know, tourism is one of our most-income business here in the Philippines. But one thing I
realized as I browse this file was that safety is not included in the development of the area. For
example is installation of the so called "bulk water dispenser" commonly seen abroad. It dispense out
gush of water coming from an upper level or due to heavy rain. It also catches trees and twigs included
in the flood giving people in the lower area time to evacuate before it totally sweeps the entire route.
Of course this includes a warning system.

:arrow: To our National Representative, I don't know how but I know there's a way to improved safety in this
kind of business. We are already given example. Hope this does not happen again.

:cry: It sooooo sad.
